Gut Dysbiosis: Understanding the Signs, Causes, and Science Behind an Imbalanced Gut Microbiome


The human gut is home to an estimated 38 trillion microorganisms, including bacteria, fungi, viruses, and archaea. Together, these microorganisms form the gut microbiome, a complex ecosystem that plays a vital role in digestion, nutrient absorption, immune regulation, metabolism, and communication with the brain.

In recent years, research has increasingly linked the health of the gut microbiome to both digestive and overall health. One term that has gained attention is gut dysbiosis—an imbalance in the composition or function of the gut microbiota.

Although gut dysbiosis is not considered a disease itself, growing scientific evidence suggests that it may contribute to a wide range of gastrointestinal and systemic conditions. Understanding its signs and the factors that influence gut health can help individuals make informed lifestyle choices.


What Is Gut Dysbiosis?

Gut dysbiosis refers to a disruption in the normal balance of microorganisms within the gastrointestinal tract. This imbalance may involve:

  • A reduction in beneficial bacteria

  • An overgrowth of potentially harmful microorganisms

  • A loss of microbial diversity

  • Changes in the metabolic activity of the gut microbiota

A healthy microbiome is typically characterised by high microbial diversity, which allows different species to perform complementary functions that support digestion, immune homeostasis, vitamin production, and protection against pathogens.

When this balance is disturbed, the gut's ability to maintain these functions may be compromised.



Why Is the Gut Microbiome Important?

Scientists often describe the gut microbiome as a "hidden organ" because of its extensive influence throughout the body.

Research suggests that the gut microbiota contributes to:

  • Digestion of dietary fibre and production of short-chain fatty acids (SCFAs), particularly butyrate, acetate, and propionate, which nourish intestinal cells and help maintain the gut barrier.

  • Regulation of approximately 70% of the body's immune cells, many of which reside within the gastrointestinal tract.

  • Synthesis of certain vitamins, including vitamin K and several B vitamins.

  • Maintenance of the intestinal barrier, helping to prevent harmful microorganisms and toxins from entering the bloodstream.

  • Communication with the brain through the gut-brain axis, involving neural, hormonal, and immune signalling pathways.

These functions help explain why alterations in the microbiome may influence health beyond the digestive system.


Common Signs of Gut Dysbiosis

The symptoms associated with gut dysbiosis are often non-specific and may overlap with many other conditions. However, common signs include:


Persistent Bloating and Excessive Gas

An imbalance in gut bacteria may alter fermentation patterns within the colon, leading to increased gas production and abdominal bloating.


Irregular Bowel Habits

Constipation, diarrhoea, or alternating bowel patterns may reflect changes in microbial composition, intestinal motility, or inflammation.


Food Sensitivities

Some individuals develop increased sensitivity to foods that were previously well tolerated. This may relate to altered digestion, microbial fermentation, or changes in intestinal permeability.


Abdominal Pain or Discomfort

Disruptions in the microbiome may influence gut motility, visceral sensitivity, and local inflammation, contributing to ongoing digestive discomfort.


Fatigue and Reduced Energy

Although fatigue has many possible causes, chronic inflammation, altered nutrient absorption, and changes in gut microbial metabolites have been proposed as contributing factors.


Skin Conditions

Emerging evidence supports a connection between the gut and skin, known as the gut-skin axis. Dysbiosis has been investigated in relation to conditions such as acne, eczema, psoriasis, and rosacea, although these disorders are multifactorial.

Mood Changes

Through the gut-brain axis, intestinal microbes produce neurotransmitter precursors and influence inflammatory pathways that may affect mood and cognitive function. Research is ongoing, but studies continue to explore links between gut health, anxiety, and depression.


What Causes Gut Dysbiosis?

Multiple environmental and lifestyle factors can influence the composition of the gut microbiome.

Common contributors include:

  • Frequent or prolonged antibiotic use

  • Diets high in ultra-processed foods and refined sugars

  • Low dietary fibre intake

  • Chronic psychological stress

  • Poor sleep quality

  • Excessive alcohol consumption

  • Smoking

  • Gastrointestinal infections

  • Certain medications, including proton pump inhibitors and some anti-inflammatory drugs

  • Ageing and chronic disease

Importantly, no single factor causes dysbiosis in every individual. Rather, it is often the result of multiple interacting influences over time.


Conditions Associated with Gut Dysbiosis

Current research has identified associations between gut dysbiosis and several health conditions. These include:

  • Irritable bowel syndrome (IBS)

  • Inflammatory bowel disease (IBD)

  • Obesity

  • Type 2 diabetes

  • Non-alcoholic fatty liver disease

  • Allergic diseases

  • Metabolic syndrome

  • Certain autoimmune disorders

It is important to note that many of these relationships are associative rather than causal. Researchers continue to investigate whether dysbiosis contributes to disease development, results from disease, or both.


Supporting a Healthy Gut Microbiome

While no single food or supplement can "fix" gut dysbiosis, evidence supports several lifestyle strategies that promote microbial diversity and gut health.


Eat More Fibre

Dietary fibre serves as a prebiotic, providing fuel for beneficial bacteria. Adults are generally encouraged to consume around 25–30 grams of fibre daily, although many people consume considerably less.


Increase Plant Diversity

Research from the American Gut Project found that people who consumed 30 or more different plant foods each weektended to have greater microbial diversity than those consuming fewer plant varieties.


Include Fermented Foods

Natural yoghurt with live cultures, kefir, kimchi, sauerkraut, miso, and tempeh provide live microorganisms that may positively influence the gut microbiome.


Consider Prebiotic Foods

Foods such as onions, garlic, leeks, asparagus, oats, bananas, chicory root, and Jerusalem artichokes contain prebiotic fibres that nourish beneficial bacteria.


Exercise Regularly

Regular physical activity has been associated with greater microbial diversity and improved metabolic health.


Prioritise Sleep and Stress Management

Sleep disruption and chronic stress can alter gut microbial composition through hormonal and immune pathways. Consistent sleep habits and stress-reduction techniques may help support a healthier gut environment.


Use Antibiotics Responsibly

Antibiotics remain essential for treating bacterial infections, but unnecessary use can temporarily reduce microbial diversity. They should only be taken when prescribed by a healthcare professional.


When Should You Seek Medical Advice?

Digestive symptoms should never be self-diagnosed as gut dysbiosis alone.

Consult a healthcare professional if you experience:

  • Persistent abdominal pain

  • Blood in the stool

  • Unexplained weight loss

  • Ongoing diarrhoea or constipation

  • Difficulty swallowing

  • Severe bloating that worsens over time

  • Symptoms lasting more than several weeks

These may indicate conditions that require medical assessment and treatment.


Could Metagenomic Microbiome Testing Be Right for You?

Unlike traditional 16S sequencing, metagenomic testing analyses all the genetic material in a stool sample. This provides a more comprehensive picture of the microorganisms present—including bacteria, fungi, viruses, and archaea—and offers information about the functional potential of the microbiome, such as fibre fermentation and short-chain fatty acid production.


While these insights can help identify patterns that may support personalised nutrition and lifestyle recommendations, it's important to recognise that microbiome testing is not a diagnostic test and should not replace a medical evaluation. Results are most meaningful when interpreted alongside your symptoms, medical history, diet, lifestyle, and, where appropriate, other clinical investigations.


In my practice, I use metagenomic microbiome testing as one component of a comprehensive assessment. Rather than treating the test in isolation, I combine its findings with a detailed health consultation to develop evidence-informed, individualised recommendations that support long-term gut health.

The gut microbiome is one of the most exciting areas of modern medical research. While scientists continue to uncover its complex relationship with human health, it is clear that maintaining a diverse and balanced gut microbiome supports healthy digestion, immune function, and overall wellbeing.

Although gut dysbiosis is not yet fully understood, adopting evidence-based lifestyle habits, including a fibre-rich, plant-diverse diet, regular exercise, adequate sleep, stress management, and appropriate use of medications can help create an environment where beneficial gut microbes thrive.

As research evolves, our understanding of the gut microbiome will continue to shape the future of nutrition and preventive healthcare, reinforcing the idea that good health truly begins in the gut.
